Job Details

• LPN position provides 6 patient visits per day and works within teams of RNs, PT, OTs, HHAs.
• Responsible for performing skilled nursing functions in accordance with the State Nurse Practice Act and acceptable standards of practice, federal, state, and local laws and within the guidelines of professional license.
• Delivers care to patients as delegated by the RN Case Manager and/or Clinical Team Manager.

Job Requirements

• Home care experience required.
• Primary Source Verification of professional license and/or certification within 30 days of start date and updated upon expiration.
• MA Conditional Licenses not accepted.
• Both NURSYS and BON are required for nursing licenses.

Schedule Information

• Monday – Friday 8am -4:30 pm with weekend rotation of 1 /4 weekend requirement.
• Required to work one weekend shift per month.

Additional Details

• Fit Test (N95) within 365 days to start date and updated annually.
• Driver's License/State ID required.
• MGB Badge Photo required.
• MGB MVR Check required within 45 days of start for all home care positions.
• Documentation of MMR vaccination or immunity required.
• Influenza Vaccine required during flu season (Sept 15th - April 30th).
• TB Test documentation within 3 months of start date and updated if >6 month break.
• Health Statement signed by MD/DO/NP/PA within 365 days of start date and updated if >6 month break.
• MGB Criminal Background Check within 45 days prior to start date and updated if >6 month break.
• MGB Massachusetts CORI within 30 days prior to start date and updated if >6 month break.
• MGB Resume including past 7 years of work experience required for submission.
• MGB References: 2 recent references, minimum 1 Supervisor or Above, in Skillset.
• MGB Skills Checklist required for submission.
• MGB Confirmation of Assignment/POI Form required for all initial assignments and extensions.
• Respirator Medical Clearance (RMC) Attestation required if fit test is required for skill.
• MGB Education Verification through background check company.
• MGB SSN Trace must be run with Criminal check.
• MGB National Sex Offender Check ran within 30 days of start and updated annually.
• MGB Exclusion Checks ran within 30 days of start and annually.
• MGB Candidate Submission Coversheet required for submission.
