Responsibilities: + Assists with implementation of the domestic plan of care which may include but is not limited to; preparing meals, running errands, house cleaning and doing laundry. + Maintains accurate documentation in accordance with policies and procedures. + Communicates participant concerns or changes in participant status to supervisor. + Home Health services may be provided in a variety of locations including but not limited to participant residence. **Requirements** Education: + High school diploma or GED preferred. **Additional Preferences** + BLS Provider.
